al kisah..

Berlalu sudah 3 wk surgery posting yg sgt memenatkan but sgt menyeronokkan!!..so tinggal 2 wk lg b4 end of posting exam..wah cepatnya masa berlalu..rasa 3wk ni sgt cepat..uh i really like surgery posting so much..huhu..dulu pernah simpan angan2 nk jd surgeon..but rase nye cam xkesampaian coz i realized dat i hav tremor bile start pegang operation stuf.huhu..espcially time nk wat suturing..sgt obvious tremornya~~ wawawa..seday..mayb xcukup practice kot..hopefully later can overcome diz prob.hhehe...amin

So far after 3 wk..byk perkara2 baru yg saya belajar...Antaranya diz time baru terasa terexpose to so many interesting cases in the ward ..byk clinical2 skill yg di improve kan..dulu time phase 3A cam terkial2 je bile nk wat examination..then the most important thin' is we learn how to manage the patient..huhu.. terasa wah byk nye improvemnt..but still got some more to improve..lacking here and there..ermm hopefully sempt cover b4 final exam...amin

Antara perkara2 menarik during diz surgery posting:

- 1st week

da kena present bedside isk2: so clerk la sorg patient ni..initially presented wif history of left upper limb swelling bout 1 month..asal2nye die kena needle stick injury time menjahit jer..pas2 makin teruk pas die g urut kat bomoh..i'm not saying dat traditional med is not good..so bile da teruk sgt..sampai xboleh angkat tangan n sakit yg sgt kuat baru die g hospital..then admitted.after investigated found out there's subclavian vein thrombosis..treated wif anticoagulant sume laa kan..after discharge pun makcik tu ckp tangan die still bengkak gak..1wk je ok..pastu bengkak tu makin teruk..then finally developed shortness of breath..masuk blk hospital 4 2nd time..2nd day die admitted laa sy clerk makcik ni..time 1st jumpe time..die cam xcomfortable..mngadu sakit sume..tangan xle angkat..jari2 xle gerak..kesian sgt..bile sy cek case note..rupe2 this admission doctor notice ade lump kat left breast die..then ada beberapa lymph node enlarged gak below axilla..so agak susah mula2 nk amik history from makcik ni..coz die cam look so ill n depress..but after lame gak rapport..baru dpt examine..dat good for me coz i can appreacite the findings..but not for diz makcik coz the next 2 day FNAC result showed that the lump is a cancer..n more specific is an infiltrating ductal breast ca..kesian kan? semoga makcik tu tabah menghadapi dugaan dunia..moral of the story--> plzz to all my fren ( girl only huhu) do breast self examination monthly best time is 1-2 week after 1st day of period!..as simple as 5 steps..(if dunno juz ask me..huhu cam terer laa kan or can search from internet also can!).. prevention is better than cure rite..so mengingat utk diri sendiri juga and buat anda sume

- 2nd week

present tutorial pun bout breast CA.. then time short case pun dapat case breast lump..my groupmate sampai ckp cam da jd specialist breast..uh hakikat tidak~~ terasa takut pula utk diri sendiri..memikirkan incidence (newly diagnosed breast ca cases) increase in number espcially in M'sia..u know why?? late age of 1st pregnancy..meaning wat?? dapat anak lambat bile umur da lanjut..biasanya or more risk if age more than 35..u know why?? adakah perempuan2 skrg susah utk bertemu jodoh or wat?..may be~~..mayb perempuan2 skrg lebih pentingkan carier sampai kawin tu tidak penting bg mereka..so terjadi laa kawin lewat..so dpt anak pun lewat.. so nasihat utk diri sendiri and for all women out there..thin' about it..plzz if da ade jodoh tu kawen cepat2..jgn tunggu2 lg..and cepat2 wat ank.hehe..n bg sesapa belum de jodoh lagi..jgn wat bodo..trry le cr pasangn yg sesuai..agar kita xtergolong antara org2 yg high risk for breast cancer..ok?

- 3 wk :

adalah wk yg plg letih..penat skali..4 hari berturut full with tasks to setel..2 tutorial kena present..monday: kena present management head injury..tuesday :bedside wif mr.cheong..ni best coz doctor trained us liked HO..clerk sume patient kt ward..then present brief history..then discuss management briefly..agak penat laa..coz kul 8 jer da kena g ward nk clerk..sorg cam kena cover 5 beds..pas2 bile time bed side bayangkan laa..gi one by one patient...uh nak abeskan more than 30 beds in ward..x ke kejang kaki berdiri..so dr kul 8-10 clerk..11-1 lbh bedside.uh mmg fatigue sgt after dat...xpela..blaja punya pasal..the best thin' for dat day is..for the 1st time i see this xray..so can u guess what's the finding in diz xray??

Answer: air under diaphragm (right side)

- wednesday:

ni pun hari yg agak menarik..coz kitorg kena follow plastic surgeon..actly diz 2nd class kitorg..1st time 1st wk..bile ckp plastic surgeon jer ( no need to mention the name) sume cam cuak gak..coz dat doctor mmg agak 'scary movie' sket..huhu..coz muke cam serius giler..but so far ok jer die..not so bad..byk gak dia ajar..alot i learnt from him.. plastic posting quite intersting..huhu..yg plg menarik nye dat day..ingatkan xyah clerk case for bedside..last2 ttiba jer doctor soh clerk one patient time kat clinic..so bcoz mmg da my turn 4 clerking so pasrah jer laa.. ala2 HO style laa kan..clerk n examination ala2 less than 30 min i thin so..uh gelabah gile..coz mmg xprepare history sume terus present..da laa blurr giler..xtaw case ape..paham2 je laa case plastic kan pelik2..mmg terbukti case yg sy clerk tu pelik..n seriusly xpernah dgr..doctor sendiri said dat only 1-2 case a year..so juz imagine..how bad my presentation?? huhu..dgn history yg x sempt manage..physical examination yg hampeh..diagnosis xyah cakap laa kan..sure laa xtaw!!..so nk taw case ape?..hav u heard about AMENOBLASTOMA?? dat good if u know..so sad for me coz i really dunno..uh akan ingat sampai mati kes ni..briefly bout this case..a young gentleman aged 20 presented with painless lump over the right jaw..progressively increase in size bout 1 year till the lump became so big enuf to cover half of his face..associated wif gum bleeding..pus discharge wif foul smelling sume laa..pas2 bile doctor tunjuk xray ape ntah name die xingat..huhu..ade embedded teeth kat right mandile with bone cyct if xsilap..uh serius..mmg bile kena descibe xray tu mmg blurrrrr..xtaw nk descibe ape..sebaik laa doctor explain sume..br laa taw ape ke benda nya tuh!!!..good for us know a new thing.. the good thin' is..operation yg patient tu went thru.. for me it's a success operation laa..coz mmg reconstructive yg pastic surgeon wat tu cantik..muke patient tu cam like normal person..actly patient underwent removal of right mandible..n replace with bone flap from iliac creast..u know how long the operation?? 14 hours if im not mistaken..lame gile kan?? but the result soo amazing..itu laa hebat nya plastic surgeon!! huhu~~

- thursday:

1 more tutorial i need to present on dat day..serius dat day mmg da fatigue gile..b4 presentation tu da sakit kepala sgt2..coz rasenye xcukup tido ciapkan slides presentation mlm tuh..mmg xsabar nk abes presentation..coz boley merdeka daa..after tuto sempt lg g ward teman kak fatimah clerk case for bedside esoknya..balik tuh..mmg terus pengsan..qada' tido laa..tau brapa lame?? 11 hours..yeh! puas hati..haha

-friday:

setelah berapa lama try to observe mamogram xdpt2 coz timetable yg pack..so td berjaya juger..ermm..after observed dgn mata kepala sendiri..baru taw camne..the procedure i juz can say dat it's a very uncomfortable .. but no doubt to do it.. uh also new experience for me today..thanks god coz give me this opportunity..n make me to think dat..sy sepatutnya sgt bersyukur coz diberi kesihatan yg baik..and hopefully dijauhkan dari segala penyakit2 chronic..amin

so..after everythin setel for diz wek..relak kan diri..coz cam da lame xtgk movie..so sgt terhibur sgt td dpt tgk Bolt..so sweet laa citer tuh...uh akhirnya..tiba juga weken hari utk berehat!! yeh~~



so..another 2 wk to go! caiyuk~~
